New Provider Documentation

To ensure a seamless onboarding experience, all new providers are required to review and complete all New Provider Documentation (links below). Use the New Provider Documentation Checklist to track your progress and confirm completion.

Follow this page for a detailed guide to completing all requirements for new providers.

Note: Missing any of the following deadlines may result in start dates being pushed back 30-90 days.

Note: Physicians, if you have an out-of-state DEA license and/or Medical license, update your license to reflect your practice in Ohio.

  • Complete Modio Profile - Deadline: Five days from receiving the Modio invitation

    • An email containing a link to Modio will be sent to the personal email you provided to the credentialing team. The email with the link will provide a detailed overview of how to complete the profile.

  • Complete OhioHealth Network Application - Deadline: Three weeks from receiving the DocuSign application

    • Please complete the information not already provided in the prefilled application. Note that there are timely requirements for finishing a TB blood test and completing fingerprint screening.

Once the OhioHealth application is complete and submitted, a representative from OhioHealth credentialing will contact you via email stating that the application is complete/incomplete. This will start the next process of correcting errors or credentialing per practice location, for example, Riverside Methodist Hospital, etc.

  • TB blood testing information

    • Central-Ohio Resident – Complete the testing at an OhioHealth Associate Health and Wellness (AH&W) location. AH&W locations are listed in the table below. Please complete the "OhioHealth Lab and Requisition and Release Form." AH&W will complete the order for your TB blood test, and you will provide the order to an OhioHealth lab to complete the blood draw. Please obtain a copy of your results and forward them to the credentialing team.

    • Out-of-state or Non-Central-Ohio Resident – TB blood testing can be completed at a participating location at Jasonhealth.com. Please follow this link to find a facility in your area. Once you have received the results please send them to the credentialing team.

    • Testing center locations and hours.

*If you have had TB blood testing completed within a year of the OhioHealth application date, please submit a copy to the credentialing team for review. A credentialing member from MPAS will contact you for approval/denial of testing requirements.

  • Fingerprint screening information

    • State of Ohio Resident - Make an appointment with FastFingerprints in the state of Ohio:

      • Go to the following website: FastFingerprints

      • Select "I HAVE A CODE":

      • Enter the following Access Code: 12KEV2S8

      • Select "Verify Code"

      • In response to "Was your code provided by 'OhioHealth'?", select "YES"

      • Enter a convenient zip code and mile radius and click "SEARCH"

      • Choose the location and click "SELECT LOCATION" and then "NEXT"

      • When prompted, complete the following:

        • Schedule a Time

        • Enter the required Applicant Information

        • Verify/Confirm information, Sign, and Submit

* At the time of your scheduled appointment be sure that you bring a valid, government-issued photo ID (driver’s license, state ID, passport, military ID, green card) *

Provider Signature

o   Please follow this Link and complete the directions listed below

  • Log into I & A (use your NPPES log-in/password)

  • On the HOME page, click the MY CONNECTIONS tab

  • Under the “Search by” is a box and your name

  • Click the “plus sign” inside the box

  • Click “Add Surrogate”

  • In the NPI field type:  1598703951 and click “search”

  • Under “Search Results” should be Midwest Physician Anesthesia Services, Inc.

  • Click on the circle next to Midwest Physician Anesthesia

  • Then check the boxes for: PECOS and NPPES

  • Click Continue

  • Click Submit

Once hospital approval is completed you will have OhioHealth training to complete that will be titled under OhioHealth University. This training will be completed onsite at an OhioHealth location.
